Parents/Guardians: PLEASE READ AND SIGN. BY SIGNING BELOW, I ACKNOWLEDGE THAT:

I have received and will make myself familiar with a copy of the studio brochure, which contains a description of all rules, regulations, dress code and yearly calendar for the studio.
I understand that if the student is one month delinquent on tuition, that student cannot continue lessons until the balance is current.
I understand that tuition is due by the 1st of every month. Any tuition turned in after the 15th day of the month, will have a $10 late fee applied.
I understand that any check returned by the bank will have a $25 returned check fee applied to my bill. Once a check is returned, the situation will be resolved by cash only.
I understand that the student enrolled will be respectful to authority and property. Also, student must be respectful to fellow students verbally and physically. The studio reserves the right to terminate your enrollment without any refunds on fees or tuition.
I agree to have the enrolled student measured by staff and costumes tried on by staff. All measurements and information are kept confidential.
Parents need to come in the studio to keep good communication with teachers and events.
NO UNSUPERVISED CHILDREN IN THE WAITING AREA.
NO OBSERVERS IN CLASS.
CENTER STAGE DANCE STUDIO IS NOT RESPONSIBLE FOR LOST OR STOLEN ARTICLES.

I understand that if at any point myself and/or child decides to terminate a class, that I will be charged tuition until I NOTIFY the teacher. I understand that I am responsible for any tuition charges up to and including the month that I quit.
WAIVER: Any student participating in class takes certain inherent risks. This includes, but is not limited to sprains, bruises, pulled muscles, and broken bones. The enrolled student is responsible for any incidents pertaining to any medical conditions such as heart conditions, diabetes, hypertension, epilepsy or any other medical condition. Participation in class indicates the acceptance of such risks by enrolled students. Therefore, I, the undersigned will not hold Center Stage Dance Studio or any of its directors or employees responsible for injuries, illnesses, or damaged sustained while participating in any activity related to acrobatic class at Center Stage Dance Studio, 4837 Union Road, Cheektowaga, NY 14225.

Center Stage Dance Studio directors or employees are not responsible or liable for any harm or injury occurred on the premises and surrounding perimeter of the studio. I agree that by enrolling my child or myself at Center Stage Dance Studio, I am responsible for all cost (tuition and fees) up until the time that my child or myself enrollment has been terminated either by myself or Center Stage Dance Studio. I also acknowledge that I have received from the studio a copy of the rules and explanation of cost. We reserve the right to cancel or change class days and times.
